Advanced Family Surgery Center (AFSC), also known as Surgery Center of Oak Ridge, LLC, reported a network intrusion that was discovered on or around November 26, 2025. A third-party forensic team investigated the incident and concluded that a limited amount of protected health information may have been accessed without authorization. AFSC issued an official notice via PR Newswire/Morningstar on May 4, 2026.
While the exact number of affected individuals has not been publicly disclosed, the compromised data may include first and last name, address, date of birth, date of service, health insurance information, medical diagnosis information, medical record number, Medicare/Medicaid number, patient account number, prescription and treatment information, provider name, and Social Security numbers. There has been no indication of misuse of the information at the time of the notice. The breach appears to primarily affect individuals in Tennessee.
