WellPoint (Independent Clinics of Washington, Elevance Health) experienced a data breach on June 24, 2025, due to a phishing incident that led to unauthorized access of an email account at Independent Clinics of Washington (ICW), a vendor. The incident was reported to the U.S. Department of Health and Human Services (HHS) on October 7, 2025. ICW notified WellPoint of the breach on March 2, 2026. Individuals were directly notified.
