An unauthorized individual gained access to this account on or about October 22, 2025. The company notified the Massachusetts Office of Consumer Affairs and Business Regulation on May 1, 2026, and publicly disclosed the incident on May 4, 2026.
The breach may have involved sensitive personal and medical information, including full names, Social Security Numbers, dates of birth, addresses, driver's license numbers, medical treatment and diagnostic information, medical record numbers, and health insurance information. While the total number of affected individuals has not been publicly disclosed, a report to the Massachusetts Office of Consumer Affairs and Business Regulation indicates that 33 individuals in Massachusetts were affected.
