An unauthorized actor gained access to their systems and exfiltrated data between December 23-24, 2025. The company discovered the breach on December 24, 2025, and began notifying affected individuals around February 2026.
The compromised data varies by individual but may include patient name, date of birth, doctor name, medical record number, date of death, email address, treatment information, diagnosis information, health insurance information, Social Security numbers, and government-issued identification information. Over 3.1 million people were impacted, with at least 174,837 individuals in Texas alone. The company is providing complimentary credit monitoring services to affected individuals.
