On December 24, 2024, Community Health Northwest Florida (CHNF) discovered suspicious activity on its network, which was later determined to be unauthorized access to certain files. CHNF is doing business as Escambia Community Clinics and identified that sensitive personal information and protected health information (PHI) had been accessed.
The breach may have involved data types including names, dates of birth, Social Security numbers, driver’s license or state ID numbers, financial account numbers, credit/debit card numbers, patient ID and medical record numbers, medical information, and health insurance information. The number of individuals affected remains undetermined. Notifications to affected individuals began on January 26, 2026, through website notices and mailed letters, and CHNF filed notice with the Vermont Attorney General on January 27, 2026. CHNF has also secured its systems, engaged cybersecurity experts, and enhanced safeguards.
